Other Psychological Conditions Re-Evaluation

What is it?

This evaluation provides a current update to a previously diagnosed mental health condition. 

What does it include?

• Records review or conversation with a previous therapist to discuss diagnosis and symptoms
• Psychological Testing and Report
• Completion of any forms needed to request academic accommodations if a diagnosis or qualifying symptoms are present.

Are there any exclusions?

• This evaluation is not intended for students who have never been diagnosed, or do not have a history of mental health care. It is not a full psychological evaluation, but an update to existing conditions.
• We do not evaluate for substance abuse issues, or provide documentation for any addiction concerns.
• We do not provide evaluations for court purposes, custody disputes, parenting concerns, pending or open child protection cases, anger management, violence or any offender issues.

Fees

• Insurance will not cover the assessment as it is for academic purposes and does not meet the purposes of medical necessity.
• Fees are self-pay for $650 .
•  If you fail to show for your scheduled appointment or if you cancel less than 24 hours prior to your appointment, a late cancel fee/no show fee of $150 will be deducted from your overall payment, as your evaluator will have reserved this time for you on her schedule.
